RN Case Manager - PRN Positions

Responsible for providing social service and discharge planning functions for the hospital. Provides resources for inpatients, outpatients, and Emergency Department patients
Monitor for quality and type of services delivered to patient(s).
Ensure management of quality of care within established parameter.
Manage the delivery of patient service(s) within the contract, DRG, or per diem negotiated.
Complete(s) case management portion of interdisciplinary patient assessment within the established time frame.
Determine and prioritize discharge plan(s) in conjunction with patient/family/team goals.
Complete assigned duties within established time frames.
Maintain documentation including but not limited to team conference, patient/family concerns.
Assures team goals are documented and available for team review.
Perform initial orientation to patient and family within 24 hours of admission to discuss the rehab process, schedules, plans, and conferences.
Chairs/Co-Chairs team conferences as directed and documents plan in medical record.
Anticipate potential problem situations and intervenes to offset adverse impacts.
Demonstrates critical thinking/problem solving skills in daily activities.
Review goals with patients, family and third party payor intermittently.
Update external case manager/payor source regarding progress, plan and patient future needs within established time frames or as requested by the payor source.
Appropriately asses the need for and arrange financial resources for the patient to include federal, state, parish and local programs.
Perform timely re-certifications as needed.

Surgical Tech - FT and PRN

Schedule is varied and will assist for some weekend coverage and heavy workload periods. We have a part-time and PRN positions available to assist with weekends and after hour call schedule.

Responsible for performing generalist testing duties in the laboratory. These duties include all tests performed on-site in the areas of hematology, serology, urinalysis, chemistry, coagulation, blood gas and blood bank.
Other responsibilities include processing, storing and referencing out specimens

Requirements and Qualifications

*Bachelor of Science Degree and completion of a certified Medical Technology program
*ASCP or equivalent certification is required.
*At least 1-2 years prior experience in a laboratory preferred

Quality/PI/Nurse Educator Full-time position!

The Nurse Educator/PI/Quality Coordinator will provide planning, direction, coordination, management and supervision for the Education, Performance Improvement, and Quality Departments. This includes, but is not limited to, staff education and training, ensuring that staff training needs are met, developing policies and procedures, JC readiness, risk management functions, preparation for the SSPI committee meetings, investigation and reporting of nosocomial infections, and infection control and prevention in-servicing. By the authority of the SSPI Committee, may institute collection of cultures and initiate appropriate isolation precautions.
Will direct new employee clinical orientation, annual competency training and special training as needed.
Must have knowledge of Joint Commission standards and experience in developing and implementing clinical training programs.

Reviews prescriptions issued by physicians or other authorized prescribers to assure accuracy and determines formulas and ingredients needed.
Monitors all posting onto the patient profiles in order that accuracy, drug allergies, drug interactions, duplicate therapy, rational drug therapy and incompatibilities may be realized and better patient care received.
Checks medications prior to delivery to patient care areas to ascertain that the correct drug, dosage form, schedule and strength is being dispensed.
Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
Prepares IV admixtures and extemporaneous parenteral products under aseptic conditions.
Directs Pharmaceutical Services personnel engaged in admixing, packaging and labeling pharmaceuticals.
Supervises 2-3 Pharmacists and Techs on a daily basis

Requirements and Qualifications

Follows all infection control practices including handwashing techniques, Standard Precautions and isolation precautions.

Requirements and Qualifications

·High school diploma or general equivalency diploma (GED), preferred.
·Prior housekeeping experience and/or training, preferred.
